Program Year 2021 Application Materials
Note:
The application packet for the 2021 program year is now available for
downloading from this page. To have the complete packet, you should download
one of each document. Be sure to download the rules and statutes as well.

prefer a hard copy.
Applications for the 2021 program year will be accepted for filing any time
after 8:00 AM, October 5, 2020. To be eligible to participate in the lottery,
the complete application must be filed by 5:00 PM, October 20, 2020.
Applications received after October 20, 2020 will be processed in the order they
are received. A FAX will
not be accepted for filing.
When filing the application, submit ONE original and ONE copy of the
application form. Please send only ONE set of the required attachments.

If using population figures to calculate the eligible amount for an MRB or MCC
application, use ONLY the following publication for the source document: U.S.
Department of Commerce, U.S. Census Bureau,
2010 Decennial Census.

An officer or member of the board of the issuer must sign
the application form. The authorized representative may not sign the
application form, unless they are also an officer or a member of the board.
